The Faculty Information Form (FIF) is an optional supplementary form that allows you to describe some of your perspectives about your course. It allows you to provide information that might be important to providing a meaningful interpretation of students' responses. It does not affect or change the scores on your Report to include your adjusted averages.
The FIF is accessible from the evaluation list. You can complete it any time after the Teval has been created until the report is available.
